Form Dashboard

If a user has the required permissions, they can view the read-only Form dashboard page, which displays packet data in graphic format on the Activity Dashboards.

To set up the required permissions for this page:

• Under Form Design on the Form Group Detail/Edit page, select the Include Form Group in Snapshot Display checkbox.
• Set user permissions via Global Permissions on the Permission Group Detail page. The setting for the Form dashboard should be either Read/Write or Read Only.

The following date range options for displaying data on the Form dashboard page are available:

• This Month (the default)
• Last Month
• Last Quarter
• Year to Date
• Last Year
• Last 2 Years
• Last 3 Years
• Custom Date Range (use of calendar widgets is required)

NOTE: You must click the Refresh button after changing your date range selection.

The following chart types can be selected:

• Column (the default)
• Bar
• Pie
• 3D

Below are examples of the Forms Groups and Root Cause dashboard charts.

NOTE: The Form Group dashboard charts will display in the order listed on the Form Group List page.

Root Cause data will display for any form group that is set up to capture root cause data. When displaying the Root Cause graph, the default is to combine all Form Groups into one Root Cause. If you would like to display the Root Cause graph independently for each Form Group, simply select the packets you would like to display in the drop-down list provided and click the Refresh button.

Clicking the bars on the charts will display the individual data records in the Form Group Detail page, on which you can:

• Sort columns
• Print results
• Link to the packet record
• Use navigation

Default for the Root Cause Graph is all Form Groups where Root Cause has been identified. Use the Form Group drop-down list to select specific Form Packets when displaying the Root Cause graph.
